Work Permits

Students interested in obtaining a work permit should download the Work Permit Form and follow these steps to secure a work permit:
     1) Print the form and complete the student section. 

• Must include your Social Security number
• Must have student and parent signatures 

     2) Give the form to the prospective employer and have them complete and sign their signature. 

• Employer must return the worksheet form to the student for submission to school. 

     3) Email the completed worksheet, copy of your student ID or drivers’ license to: Vickie Gilmore at vgilmore@nmusd.us
